1. What is the product causing allergy?

2. Severity/degree of your Allergy (Minor or Major)

3. Will you still be affected if the other guests on board the airplane consume the product to which you have allergy? Please provide some details.

4. Make sure you have submitted your request no later than 48 hours prior to your flight.
